Job Description

Under the administrative direction of the Clinical Manager/Designee, the Behavioral Health Care Manager (BH CM) for Senior Care Options (SCO) is responsible for consultation and guidance on the behavioral health needs of identified high risk members. The BH CM directly interfaces with members of the Interdisciplinary Care Team, medical and behavioral health providers (internal and external), members, and caregivers in identifying risk factors, conducting assessments, and developing and implementing care plans specific to behavioral health needs.

KEY RESPONSIBILITIES/ESSENTIAL FUNCTIONS

Consultant and clinical behavioral health resource for the SCO team:

Assesses identified members' behavioral health care needs and collaborates with SCO team members and health care providers to develop a plan of care to improve, maintain, and/or support optimal behavioral health and wellness.
* Collaborates with SCO Interdisicpilinary Care Team (ICT) to provide consultation for members with behavioral health needs
* Participates in the intake and evaluation process by conducting an initial behavioral health assessment and ongoing behavioral health assessments as needed.
* Assists members of the ICT in understanding the social, emotional, cultural, and psychosocial aspect of participants' situations and the actual or potential effects on members health status and outcomes.
* Collaborates with SCO ICT members to develop individualized plans of care for high risk members.
* Assists the SCO ICT in development and monitoring of the plan of care to ensure it includes specific interventions and measurable outcomes drawn from the most recent assessment.
* Serves as a bridge for the member during and following an inpatient psychiatric stay in order to advocate for proper utilization of services and a safe discharge plan back to the community.
* Utilizes solution-focused, behavioral modification, and motivational interviewing techniques to maximize health outcomes.
* Facilitates communication between medical and behavioral heath providers and uses coaching techniques to empower members active participation in their care.

* Communicates and consults with the members of the SCO Care Managment team, Tufts Health Plan's Behavioral Health Department, as well as other cross functional areas to recommend and coordinatecare for complex member issues.
* Facilitates communication with external behavioral health providers, state agencies, ASAPS, primary care offices and inpatient and outpatient behavioral health services.
* Develops and maintains relationships and expert knowledge of geriatric and community based resources, providers and agencies.
* Facilitates and/or participates in the coordination of ICT meetings and other forums to review members' plans of cares.
* Performs all duties in accordance with Tufts Health Plan policies and procedures.
* Consistently follows professional Code of Ethics that corresponds with professional licensure.

Demonstrates the ability to accurately document information that reflects and supports communications and decisions. Examples of expected documentation include notes, policy reviews, letters, meeting minutes and clinical reviews.

Participates in staff meetings as scheduled. When needed, represents the SCO Care Management team in special committees and meetings.

Participates in SCO Care Management projects assigned by the SCO Clinical Manager/designee.

Performs other functions of the SCO Care Management Team as assigned.

Job Requirements

Education Requirements:
Master's degree in Social Work required, Massachusetts's current licensure as a Clinical Social Worker (LCSW) required, LICSW preferred.

Experience:
Five years of experience in geriatric case management is required. Additional one to two years managed care experience is highly preferred in an area such as; Senior Care Options, Council on Aging, or PACE program.

Other Requirements:
* Ability to travel as needed to member's homes, hospitals, skilled nursing facilities, PCP office practices, and other sites where patients receive care.
